Dear Patients,

By this letter, I inform you that I will be closing my practice effective June 30, 2026. This decision was made after careful consideration. It has been a privilege to take care of you and your family over the years. I want to sincerely thank you for the trust you have placed in me throughout my years of practice.

To ensure continuity of your care:

The access gateway for orphaned patients will assign you a new family doctor.

Please note that you do not need to register with the access gateway to get a new family doctor – your registration is already active in GAMF (Guichet d’Accès aux Médecins de Famille).

You may obtain a copy of your medical file by submitting a written request before June 30, 2026.

Your electronic medical records will be retained in accordance with the legal and ethical requirements of the Collège des médecins du Québec.

Continue to take care of your health – it is very precious. I wish you all the best for the future.
